Broker Certification/Re-Certification Registration Packet/Forms: Prior to bidding on a HUD home, the Broker submitting the bid must obtain a NAID number. NAID numbers can be obtained by submitting the following packet of forms and documents. Faxed copies cannot be accepted and original signatures in blue ink are required. Beginning 12/13/07, a revised version of form SAMS-1111, Payee Name and Address, (dated 12/07) is available. The most significant change to this form is the requirement of IRS documentation, rather than IRS form W-9, to verify the vendor name and Tax Identification Number (TIN). This change will permit HOC staff to verify that the Name and TIN entered on the SAMS-1111 matches exactly. The form instructions provide some examples of acceptable IRS documentation.

2. Copy of a Utility Bill or Bank Statement showing the physical address of the office. NO PO BOX WILL BE ACCEPTED.

Sales Contract Packet Submission Information: The sales contract packet must be received within 48 hours of bid acknowledgement. Bid acknowledgements are posted under “Bid Results.” The sales contract packet includes the HUD-9548 Sales Contract, addenda, attachments, copy of earnest money, copy of prequalification letter and/or evidence of cash, and a self-addressed business envelope.
